Commit to Be Fit Wellness Program
Commit To Be Fit is an innovative worksite wellness program that yields a healthier, more productive workforce while helping to control medical costs. The program, which utilizes a preventative and disease management approach, was developed collaboratively by SOURCE member employers, MorningStar Health, and Spectrum Health Healthier Communities. Our on-site services will educate individuals about their health, the most serious chronic disorders, and how the health care system works (purchasing, use & costs), so that we all will better understand how our lifestyles impact the health care system. Commit To Be Fit incorporates a paradigm shift toward consumer-driven health plans by providing goals, coaching, and incentives so that we may be more accountable for our own health and reduce our risk of developing chronic disorders, such as diabetes, heart disease, hypertension, asthma and depression.
OUR GOAL: To improve and maintain the overall health of our employees while controlling healthcare costs and positively impacting productivity.
PILOT DESIGN
- Portable - The Wellness Program is designed to work with any employer's health plan
- Confidential - Third-party administration ensures confidentiality of personal health information
- Collaborative Health Management - Coordination and communication with primary care physicians to manage participants' overall health
- Financial Rewards for Program enrollment and achieving goals
- On-site Health Coaching by a registered nurse to help participants achieve their goals
- Health Risk Appraisals (HRA) - self-evaluation of one's own health and lifestyle behaviors complete with an individualized report
- Educational Component so participants and their families can understand the different factors affecting their health
- Consumer-Driven Health Plan Philosophy in which participants are accountable for their own health and actively involved in the decision-making of health care services
- Comprehensive Research of Group Trends conducted by University of Michigan Health Management Research Center
- Favorable Return-On-Investment (ROI) Projections from cost savings associated with reductions in health claims costs, absenteeism, short-term disability, and Workers' Compensation.
